Welcome!

We are the Diabetes Management Centre (DMC) that serves Halifax and surrounding areas.  We provide information on all aspects of how to care for diabetes.  This is tailored to people with diabetes and their interested family members as well as other health professionals.

We promote self-management to enhance your quality of life and to reduce the risk of developing diabetes complications.  We work closely with other community agencies to provide comprehensive diabetes care for our area.

We are open Monday to Friday from 8:00 a.m. - 4:00 p.m. and some evening hours.

For information about our services, please phone (902) 454–1600.  Your first appointment will be made after your doctor has referred you to our program.  The staff of the diabetes program will arrange follow-up appointments.

If you have diabetes or have been told you have a mild form of the disease, we would be more than happy to assist you in managing your diabetes.  Ask your doctor to refer you.

We provide a number of ways to learn about your diabetes such as: 

  • Individual instruction (limited, based on specific criteria).
  • Group instruction. (This provides you with an opportunity to learn from and with others who also have diabetes.)
  • Insulin start. (If you need insulin, this means that you will learn how to start taking your insulin as an outpatient.  You will not need to be admitted to the hospital.)
  • Pre-Diabetes program. (Provided to those people whose blood work indicates Impaired fasting glucose (IFG)/Impaired glucose tolerance (IGT). A group education program that will help you decrease your risk of developing diabetes and heart disease.)

Other:  We also offer a variety of Group Sessions such as Insulin Adjustment, and Diabetes Stress Management.  Call us for additional information on when and where these events will be offered.

Our satellite service is provided at the Cobequid Community Health Centre in Lower Sackville, Monday through Friday from 8:00 a.m. - 4:00 p.m.
