The guidelines produced by the Diabetes Care Program of Nova Scotia (DCPNS) have been developed as the result of an identified Nova Scotian need.

bullet The most current guidelines are listed first and you will note that some are under review/revision.
bullet The earliest guidelines were primarily aimed at Diabetes Centres to facilitate standardization in diabetes care, education, and complications surveillance and monitoring.
bullet Guidelines have been developed for special populations to ensure quality care and consistency in approach across the province. These include guidelines for pregnant women with diabetes, children and youth with diabetes, and individuals with a diagnosis of prediabetes.

Guidelines have been developed by committee with valued input from tertiary program staff as well as educators and other providers representative of Nova Scotia rural and urban practices. DCPNS GUIDELINES

Diabetes Guidelines for Elderly Residents in Long-Term Care Facilities Pocket Reference (2010)
The Diabetes Care Program of Nova Scotia (DCPNS) is pleased to announce the launch of Diabetes Guidelines for Elderly Residents in Long-Term Care (LTC) Facilities in Nova Scotia. These guidelines provide safe and standardized diabetes care (targets for glycemic control and treatment of hypoglycemia) for frail elderly residents. You may find that the recommendations in the Pocket Reference may not be the usual practice in your facility; however, please review these with the supporting document.
Copies of the Pocket Reference as well as the supporting materials have been distributed province-wide to all LTC facilities (May 2010). If additional copies are required, please contact the DCPNS office or download and copy the following documents as needed.

DCPNS Prediabetes (PreDM) Screening and Management of High-Risk Individuals (Adults) - 2008.
The intent of these guidelines is to standardize the approach to the identification, management, and education of persons with prediabetes across Nova Scotia. These guidelines include:

Guidelines for Dyslipidemia Management and Education through Nova Scotia Diabetes Centres (2007). pdf document 233KB
This guideline document discusses the Diabetes Care Program of Nova Scotia (DCPNS) recommendations to ensure a standard approach reflective of best practice for lipid measuring, recording, reporting, and educational initiatives for persons attending Diabetes Centres in Nova Scotia. These recommendations support the self-management component of chronic disease management and are consistent with national guidelines. The document includes a supporting literature review, 11 key recommendations, rationale for the recommendations, and references.

This guideline document discusses the Diabetes Care Program of Nova Scotia (DCPNS) recommendations regarding implementation of standard blood pressure measuring, recording, reporting, and educational initiatives for persons attending Diabetes Centres (DCs) in Nova Scotia. These recommendations support the self-management component of chronic disease management and are consistent with national guidelines. This relatively short document includes a supporting literature review, strong chronic disease management and the self-care focuses, 12 key recommendations, rationale for the recommendations, DC action algorithm, and reference list.

DCPNS Triage Guidelines for Initial and Follow-Up Appointments in Nova Scotia Diabetes Centres (2004). pdf document 167KB
Increasing waitlists and workloads are major challenges in Diabetes Centres throughout Nova Scotia. In Nova Scotia, the referral rates of individuals with newly diagnosed diabetes have increased 98% since 1992/93. These guidelines are aimed at ensuring timely access for newly diagnosed individuals and more effective follow-up programming (based on individual need and diabetes status) for ongoing care/education.

A comprehensive guidelines manual for the management and education of children & adolescents with diabetes mellitus and their families. Sections include: diagnosis and initial assessment (including type 2 diabetes), psychosocial considerations, age-appropriate education, adolescent issues, nutrition, exercise, insulin, pump, blood glucose monitoring, monitoring of ketones, hypoglycemia, sick days, DKA, surgery, school and day care, and documentation.

Pregnancy and Diabetes Management Guidelines Manual, 2nd Edition (2000) - includes three revised sections – Insulin, Medical Management, and Screening (2006) - pdf document 13.96MB
A comprehensive guidelines manual for the management of pregnancy complicated by diabetes mellitus (both women with preexisting diabetes and women diagnosed with gestational diabetes [GDM]). Sections include: screening, psychosocial considerations, medical management, nutrition, physical activity, insulin, blood glucose monitoring, ketone monitoring, hypoglycemia, breastfeeding, postpartum care, newborn care, and documentation.

DCPNS Initial Guideline Documents (1992)
The eight initial guideline documents noted below were released by the DCPNS in 1992 as a basis for standardized care and approaches in Nova Scotia's DCs. Programs have been measured against these guidelines in ensuing years as part of evaluative criteria, practice surveys, audits, etc. Data capture from standard forms contributes to the DCPNS Registry. These are not available, but archived within the DCPNS.
